CHC Funding for professionals

 

We have a team of Continuing Health Care (CHC) nursing experts who are able to accept instructions from professional/legal clients acting for members of the public.

CHC Expert Reports – We are able to provide reports to clients as to eligibility for CHC funding. Our CHC nurse experts are able to examine the assessments and reports and advise on the client’s eligibility status. These reports can then be submitted to the appeals panel. Our nurses are also able to attend the panel hearing/meeting if and when required.

Checklists – This is the initial phase of the process, it is important it is carried out correctly and accurately as this could determine whether a client’s case goes forward to the next phase (decision support tool) of the assessment process. Our team is able to review and advise on any checklists completed within the clinical arena.

Health Needs Assessment – Many Clinical Commissioning Groups (CCGs) require this assessment as part of their process. This document is a forerunner to the full assessment. The nursing team at Apex Health Associates are able to review the client and prepare robust health needs assessments.

Decision Support Tool – The nurses in the Apex Health Associates CHC team are skilled in the use of this document. They are able to attend the decision support meetings as an advocate for the client and to ensure the process is being followed correctly; this serves to ensure all necessary information is included and the needs are scored appropriately.

Care planning – A care plan is a document prepared for the individual client explaining their needs and the measures required to appropriately meet those needs in a community setting. CCGs will request a care plan be completed for clients whose needs are to be managed in their own home. At Apex Health Associates we can provide expert advice and guidance on this planning process.

Specialist Reports – Apex Health Associates has a network of specialist nurse experts across the UK. If your client has a particular problem that requires a specialist nursing opinion we will hopefully be able to assist by providing such an opinion.
