We have a team of Continuing Health Care (CHC) nursing experts who are able to accept instructions from professional/legal clients acting for members of the public.
CHC EXPERT REPORT
We are able to provide eligibility reports to clients for CHC funding. Our expert nurses are able to examine the assessments and reports and advise on the client’s eligibility status. These reports can then be submitted to the appeals panel. Our nurses are also able to attend the panel hearing/meeting if and when required.
CHECKLIST
This is the initial phase of the process, it is important it is carried out correctly and accurately as this could determine whether a client’s case goes forward to the next phase (decision support tool) of the assessment process. Our team is able to review and advise on any checklists completed within the clinical arena.
DECISION SUPPORT TOOL
The nurses in the Apex Health Associates CHC team are skilled in the use of this document. They are able to attend the decision support meetings as an advocate for the client and to ensure the process is being followed correctly; this serves to ensure all necessary information is included and the needs are scored appropriately.
CARE PLANNING
At Apex Health Associates we can provide expert advice and guidance on this planning process. A care plan is a document prepared for the individual client explaining their needs and the measures required to appropriately meet those needs in a community setting. CCGs will request a care plan be completed for clients whose needs are to be managed in their own home.
